For over 25 years, Southwest Care Center has been providing exceptional healthcare to the communities of Santa Fe and Albuquerque, NM. We are a non-profit, FQHC-LAL providing medical, behavioral health, case management, community outreach, diabetes education, and pharmacy services. We are proud to offer gender equity medicine, syringe exchange, and HIV/Hep C treatment, testing and other services within our communities.

Southwest Care Center is currently seeking an experienced full-time Social Service Program Coordinator. This position supports social service programs in both Santa Fe and Albuquerque, NM requiring the candidate to reside in the local area. Some remote work is available with this position.

Position Details:

Under the direction of the Ryan White Program Manager, the Social Service Programs Coordinatoris responsible for monitoring and auditing data and workflows related to the Ryan White and HOPWA programsThis includes collecting, organizing, and maintaining data in various databases, reviewing case management worksheets, and conducting audits to support program evaluation and decision-making. Additionally, this position will work closely with program staff, stakeholders, and community-based agencies to ensure compliance with program requirements and improve access to services for individuals impacted by HIV/AIDS.

Position responsibilities include but not limited to:

  • Ryan White Program Support
  • Data entry in MS Excel, Access, E2NM, CARE Ware, and Athena.
  • Manage Ryan White Client Quest Lab balances.
  • Monitor and report Ryan White QM alerts in E2NM.
  • ManageRyan White billing and balances.
  • Track Ryan White cap on charges.
  • Audit data, Ryan White Standards of Care work output and present results and report to Ryan White Program Manager.
  • Conduct regular audits and ensure documentation compliance in Athena and E2NM.
  • Identify opportunities training needs for program staff.
  • Audit agency information for compliance with program standards.
  • Assist community-based partners as directed.
  • Audit Time & Effort on Case Management Spreadsheets.
  • Work independently and as part of a multi-disciplinary team to achieve program goals.
  • Generate and update required reports.
  • Perform other job duties as assigned.

HOPWA Program Support

  • HOPWA data entry and management for MFA/HUD requirements.
  • Track HOPWA recipients’ eligibility awards.
  • Assist with new HOPWA applications.
  • Track HOPWA Grant Spending.
  • Participate in program meetings and collaborate with key stakeholders to address housing needs.
  • Identify opportunities training needs for program staff.
  • Generate and update required reports.
  • Audit data and report to HOPWA Program Manager.
  • Assist community-based partners with connecting clients to services.
  • Work independently and as part of a multi-disciplinary team to achieve program goals.
  • Perform other duties as assigned

Candidate Highlights:

  • Minimum 2 years of experience in healthcare related field with knowledge of Ryan White and HOPWA programs highly desired.
  • High school diploma or equivalent
  • Valid Driver’s License required
