Job Description

Southwest Care Center employees can answer yes to these 3 questions:

1. Do you want to make a difference?
2. Do you believe everyone is entitled to quality healthcare?
3. Do you desire to serve the underserved in your community?

For over 25 years, Southwest Care Center has been providing exceptional healthcare to the communities of Santa Fe and Albuquerque, NM. We are a non-profit, FQHC-LAL providing medical, behavioral health, case management, community outreach, diabetes education, and pharmacy services. We are proud to offer gender equity medicine, syringe exchange, and HIV/HepC treatment, testing and other services within our communities.

Southwest Care Center is currently seeking full-time Healthcare Marketing Representative for the NE New Mexico Region.

Position Details:

The Marketing and Outreach Liaison, under the supervision of the Director of Social Services, will champion and promote HIV and HOPWA housing services within the Northeast Region, as designated by the New Mexico Department of Health, which includes nine counties including remote Native American and Hispanic communities. This position actively develops community partnerships and seeks individuals or groups who can benefit from HIV testing, prevention, and treatment services and HOPWA housing services.

This role will use good judgment, initiative, critical-thinking, and effective communication at all times while being a strong advocate for the populations served at Southwest Care Center.

Essential Duties:

Work will be anticipatory and collaborative with relevant activities including, but not limited to the following:
  • Identify, target, and execute new community partnerships by working within the Northeast Region to champion and promote HIV testing, prevention, treatment, and HOPWA housing services.
  • Identify and lead community and public awareness activities (e.g., posters, flyers, billboards, social media, TV or radio announcements) that provide explicit and clear links to information about available HRSA RWHAP and HOPWA services.
  • Passionately deliver promotional activities targeting the Northeast Region of New Mexico, reaching people living with HIV who either do not know their HIV status or who know their status but are not currently in care.
  • Engage with non-medical case managers, medical case managers, housing case managers, treatment educators, outreach workers, and others directly or indirectly linked to the community in key organizations within the local communities: AIDS Service Organizations, Faith Based, ethnic organizations, and other community-based organizations.
  • Use data to target the Northeast Region of New Mexico, to those who have a high probability of reaching PLWH who have never been tested and/or undiagnosed; have been tested and diagnosed as HIV positive but have not received their test results or have been tested, know their HIV positive status, but are not in medical care.
  • Outreach is to be conducted at times and in places where there is a high probability that PLWH will be identified; it is to be delivered in coordination with local and state HIV prevention outreach programs to avoid duplication of effort.
  • Under limited supervision, recruit high-risk individuals to participate in HIV prevention programs and HIV testing.
  • Help facilitate linkage to community resources, medical care, and other HIV/AIDS-related prevention and care/treatment and HOPWA housing services.
  • Coordinates with educational institutions, faith-based organizations, Native American community programs, and Hispanic community-based organizations to conduct outreach and program development in communities disproportionately affected by HIV.
  • Participate in community and cultural events targeting high-risk individuals, distributing condoms, brochures, awareness messages, and encouraging HIV testing and promoting the HOPWA housing program.
  • Develop memorandums of agreement with collaborating agencies to strengthen referrals.
  • Prepare and submit monthly program reports, quarterly reports, and board reports.
  • Ensure all program materials are translated into Spanish and culturally sensitive.
  • Develop strong community networks and establish pathways to PrEP care to expand outreach to under represented populations.
  • Conduct community needs assessment via stakeholder mapping, identifying key influence points and developing Community Engagement Plans; mapping out pathways to PrEP, HIV testing, and HOPWA housing assistance.
  • Act in ways consistent with SCC’s ViiV Healthcare’s culture, expectations, and values.
  • Understand and follow key grant compliance and regulatory guidelines.
  • Know, understand, incorporate, and demonstrate the Mission, Vision, Values and Guiding Behaviors of Southwest Care Trinity Health and Trinity Health PACE (TH PACE) in behaviors, practices, and decisions.
  • Promote and maintain collaborative relationships with managers, peers, and customers by effectively fostering a team environment, building consensus, and resolving conflicts.
  • Attend and participate in monthly departmental meetings, scheduled trainings, in-service trainings, mandatory annual in-service trainings, and educational classes as required/needed.
  • Provide weekly reporting to the Social Services Health Partnership Director and Executive Directors with respect to activity and results for the week.
  • Working closely with the creative development, social media, paid media, scalable outreach, and reporting task managers to coordinate activities and meet deadlines.
  • Working closely with task managers on message strategy and key message development and dissemination.
  • Draft campaign work plans, creative briefs, and performance reports.
  • Provide operational and technical support as needed such as writing and/or reviewing monthly technical reports, updating work plans, etc.
  • Coordinate with subcontractors, vendors, researchers, other team members, partners, and clients to establish workflows and timelines and keep them up to date.
  • Use good judgement to monitor and address client issues in a timely manner and keep the Social Services Director senior project management apprised of activity progress and barriers.
  • Contribute to business development efforts, including proposal strategy, writing support, and development of speculative creative.
