For over 25 years, Southwest Care Center has been providing exceptional healthcare to the communities of Santa Fe and Albuquerque, NM. We are a non-profit, FQHC-LAL providing medical, behavioral health, case management, community outreach, diabetes education, and pharmacy services. We are proud to offer gender equity medicine, syringe exchange, and HIV/Hep C treatment, testing and other services within our communities.

Southwest Care Center is currently seeking an experienced full-time Pharmacist. This position is located at our Jefferson Clinic in Albuquerque, NM.

Position Details:

Reporting to the Pharmacy Manager, the Pharmacist practices the profession of Pharmacy in a community setting and participates in clinical and consultant functions, as well as some light administrative duties. They will model professionalism, meet performance expectations, demonstrate the organization’s values, and most importantly deliver patient-focused services. This role serves as a patient advocate using good judgment, initiative, critical thinking, and effective communication at all times.

Primary duties and responsibilities include:

  • Prescription processing and verification, coordination of prescription delivery, insurance claim transmission issue resolution,and workflow maintenance.
  • Perform drug utilization reviews of patient profiles to prevent drug-drug interactions, drug-disease interactions, and duplicative therapies, and to increase medication adherence.
  • Patient counseling on all aspects of medication use in accordance with state and federal laws.
  • Clinical pharmacy services such as: immunizations, Medication Therapy Management (MTM), smoking cessation, naloxone prescribing, pharmacist clinician, etc.) and maintenance of the associated certifications.
  • Prescriber consultation to ensure appropriateness of therapy and medication safety.
  • Interact with multi-disciplinary program staff to provide exceptional integrated patient care.
  • Supervision of pharmacy technicians and pharmacy interns as appropriate per licensure.
  • Communication with Pharmacy Manager, Director of Pharmacy, and Management as appropriate when concerns/questions arise.

Risk Management and Safety:

  • Interaction with other SCC departments as appropriate.
  • Documentation of all patient counseling, clinical encounters, and coordination of patient care in appropriate record.
  • Communicates with other program staff including the Department of Health, CQM, etc.
  • Consultant pharmacist duties such as policy and procedures development, licensure, and regular oversight of SCC drug rooms to ensure compliance with Board of Pharmacy rules and regulations.
  • Understanding and compliance with 340B program, DEA, Board of Pharmacy, Aid & Comfort program.
  • Answer pharmacy phone calls, return calls to patients.
  • Attendance and participation in committee meetings as assigned (ex. Pharmacy and Therapeutics, Safety, Multi-disciplinary team, Hepatitis C, Research)
  • On-call duties on a rotating schedule with other staff.
  • Ordering and return of expired CII drugs in accordance with state and federal laws.
  • Travel for training, patient care, and staff coverage as directed.
  • Assumes role of Pharmacy Manager in the event of the Manager’s absence.
  • General knowledge of and ability to perform technician duties in the case of an absence or crisis including opening/closing procedures.
  • Maintenance of a work environment conducive to excellent customer service.
  • Maintenance of a clean and organized pharmacy environment.
  • Lead and participate in regularly scheduled and ad hoc pharmacy inventory as directed.
  • Carries out supervisory responsibilities for pharmacy interns and technicians in accordance with the New Mexico Board of Pharmacy policies and applicable laws.
  • Performs other duties as assigned. Additional duties may include direct support and reporting to other regulatory agencies, as well as other compliance requirements, all to be performed in a timely and accurate manner.

Candidate Highlights:

  • Experience: Entry Level with 2 years pharmacy experience in similar role desired.
  • Education: Doctor of Pharmacy degree or equivalent.
  • License/Certification: Valid licensure by State Board of Pharmacy (Renew every 2 years with 30 hours of continuing education). Pharmacist Clinician license on hire (preferred), within 2 years of hire (required). CPR Certification. Participate in annual OSHA training and HIPPAA training. AAHIVM Certification within 3 years of hire (Harkle/Jefferson). Certification in immunization delivery, smoking cessation, naloxone. New Mexico Board of Pharmacy preceptor license within 6 months of hire.
