For more than 25 years, Southwest Care Center has been delivering high-quality healthcare to the communities of Santa Fe and Albuquerque, NM. As a non-profit FQHC-LAL, we offer medical, behavioral health, case management, community outreach, diabetes education, and pharmacy services. Our services include gender equity medicine, syringe exchange, and HIV/Hep C treatment and testing.
We are currently hiring a full-time Family Nurse Practitioner. Position Details:
- Assess and diagnose patients, order relevant diagnostic tests, and prescribe medications.
- Create and oversee treatment plans for patients, including preventive care and health maintenance.
- Educate patients and families on health issues, disease prevention, and medical treatments.
- Refer patients to specialists and other healthcare professionals as needed.
- Collaborate with other healthcare providers to ensure quality patient care.
- Maintain detailed medical records and document patient care activities per Southwest Care Center policy.
- Lead clinic staff through education and outreach, participate in quality assurance initiatives, support the training of new team members, and supervise patient care provided by provider-assigned staff.
- Engage in planning and evaluating primary medical care and service programs, including policy development and recommending program changes.
- Keep professional and technical knowledge current through continuing education or seminars related to job responsibilities.
- Participate in quality measures and chart reviews as required.
- Perform other duties as assigned.
Practice Highlights:
- Typical schedule: Monday - Friday 8-5, with 32 hours of patient care and 8 hours of administrative time weekly.
- Limited call rotation may be required outside the typical schedule.
- System-wide Athena EMR.
- Telehealth visits through Mend platform.
- Onsite care team includes Physicians, NPs, Physician Assistants, Clinical Pharmacists, Behavioral Health Clinicians, Case Managers, RNs and RN Clinic Managers, Medical Assistants, Pharmacists, Health Educators, and a Registered Dietician.
- Inclusive "We Care" culture.
Candidate Highlights:
- 3 years of experience is preferred, particularly in primary healthcare.
- Completion of a Family Nurse Practitioner certification program. Experience with HIV care is highly desired.
- Current certification by the New Mexico Board of Nursing; national certification as a Family Nurse Practitioner. AAHIVS certification is highly desired.
